খ) জন্ম নিবন্ধন সনদ/জাতীয় পরিচয়পত্র (এনআইডি) /পাসপোর্ট এর ফটোকপি (Photocopy of Birth Registration / NID/Passport).
গ) মূলধন প্রমাণের প্রয়োজনীয় কাগজপত্রাদি (লিমিটেড কোম্পানীর ক্ষেত্রে মেমোরেন্ডাম অব আর্টিকেলস) (Proof of capital(Memorandam of Article if limited company)
ঘ) দোকান/ব্যবসা প্রতিষ্ঠানের মালিকানা প্রমাণের দলিল/পর্চা/নামজারি (ভাড়াটিয়া হলে চুক্তিপত্র) (Proof of ownership of the business/shop: Document/record/mutation paper (If rented agreement paper).
ঙ) হালনাগাদ হোল্ডিং ট্যাক্স পরিশোধের রশিদ (প্রযোজ্য ক্ষেত্রে) (Payment receipt of holding tax upto current year) (If applicable).
চ) আবেদনকারী এবং প্রতিষ্ঠানের মালিক ভিন্ন হলে অথরাইজড্ লেটার (Authorized letter if applicant and owner of business are not same person).
ছ) নবায়নের ক্ষেত্রে শুধুমাত্র পূর্বে ইস্যুকৃত ট্রেড লাইসেন্স এর ফটোকপি এবং হালনাগাদ হোল্ডিং ট্যাক্স পরিশোধের রশিদ (For renewal photocopy of previous trade license and payment receipt of holding tax upto current year).
